I do not have any physical limitations, mental ailments, physical or mental disabilities that would limit or prevent my child from participating in the above-mentioned activity, and, if required, will obtain a medical examination and clearance.

I, the undersigned hereby release Australian Martial Arts Federation, Australian Kyokushin Federation, board of directors and anyone connected in any way with the Australian Open from any liability in connection with the above mentioned tournament and any injuries which could be sustained by participation in such. I fully understand that if any medical treatment is given to me, such treatment will be of first aid only and I fully waive any, and all claims for injuries and damages, if they result from such treatment, whether given with or without my express consent.
